- A valid form of identification
- If requesting a carer’s certificate, the person being cared for should attend. If that’s not possible, our pharmacists will use their professional judgment to assess the request.

- Certificates are issued for minor ailments only, within the pharmacist’s scope of practice
- Valid for up to two days of leave
- Cannot be backdated or extended from an existing medical certificate
- Not available for compassionate leave or pregnancy-related absences, unless the condition is minor
- Pharmacists cannot certify fitness to return to work
- Fees apply; this service is not claimable on medicare
